What You'll Do10
- 1Build and manage a patient panel across acute, chronic, and preventive care to deliver consistent outcomes
- 2Consult patients in office testing centers and via tele health visits to address diverse needs
- 3Partner with in office teammates and virtual clinical teams in daily huddles to coordinate care
- 4Perform in office procedures when indicated and pursue additional state licensure for virtual primary care in other states
- 5Engage in ongoing professional development through Clinical Rounds and other modalities
- 6Contribute to a culture of feedback, reflection, and continuous improvement
- 7Leverage technology to deliver high quality care and communicate effectively with patients and team
- 8Navigate uncertain clinical scenarios with confidence and patient centered communication
- 9Support a collaborative team environment focused on changing healthcare delivery
- 10Maintain required licenses and certifications in California and possible additional states
Requirements10
- 1Completed an accredited NP or PA program with a national certification
- 2In the past 5 years, practiced as an Advanced Practitioner for at least: 2 years in an outpatient primary care setting, OR 1 year in outpatient primary care plus a 1 year primary care fellowship or 1+ year in urgent care
- 3State licensed in California by start date
- 4Demonstrated passion for human centered primary care and ability to communicate with diverse patients
- 5Ability to use technology to deliver high quality care and support clinical decision making
- 6Clinical proficiency in evidence based primary care
- 7Desire to be part of a team dedicated to changing healthcare delivery
- 8Openness to feedback and reflection to gain productive insight
- 9Ability to adapt personal and interpersonal behavior to patient needs
- 10Attainment of full readiness to work in Sunnyvale with full time schedule including on site responsibilities
