Personal Care Assistant, ADL & Dementia Support
Overview
Deliver hands-on personal care to clients across assisted living, home health, and community settings, serving individuals with disabilities, dementia, and neurodevelopmental disorders. Work alongside nurses and therapists to monitor health changes, manage medications, and ensure safety. CPR Certification and Hoyer Lift skills keep clients secure. This role offers direct impact on daily quality of life, with training pathways to grow into senior care or specialized memory support.
What You'll Do8
- 1Assist clients with bathing, dressing, grooming, and mobility, using Hoyer Lifts and other transfer equipment to prevent falls
- 2Monitor vital signs and behavioral cues, reporting changes to nurses or physicians during shift handoffs
- 3Prepare meals that meet dietary plans for diabetes, renal, or texture-modified diets, and provide feeding support as needed
- 4Administer medications per care plans, logging each dose in EMR to maintain HIPAA compliance
- 5Transport clients to appointments or community outings in a wheelchair-accessible vehicle, ensuring punctuality
- 6Document ADLs, observations, and medication doses in daily care logs, flagging anomalies for the clinical team
- 7Support clients with cognitive tasks like memory games or orientation cues, applying dementia care techniques
- 8Escort clients to recreation or therapy sessions, adapting activities to their physical and cognitive abilities
Requirements8
- 11+ years of experience in senior care, disability support, or home health settings
- 2CPR and First Aid certification, current and valid for emergency response
- 3Hands-on skill with Hoyer Lifts and manual transfer techniques, including lateral transfers and sit-to-stand
- 4Knowledge of dementia and Alzheimer's care, including de-escalation and redirection methods
- 5Ability to read and follow care plans, including medication schedules and behavioral interventions
- 6Strong observation skills to spot subtle health changes, such as skin integrity or breathing patterns
- 7Valid driver's license and reliable transportation for client appointments
- 8High school diploma or equivalent; CNA or PCA certification preferred
