Pediatric HICU LPN, Hospital at Home & Infusion Care
Overview
LPN will provide ICU level pediatric home care with family training and ongoing support in the Pittsburgh area. Join a hospital at home team delivering skilled nursing to infants and young children, acting as an extension of hospital care. You will care one-on-one, coordinate with families, and help transition patients from hospital to home. This role blends high-tech pediatric nursing with compassionate home-based care.
What You'll Do10
- 1Build and maintain high-quality pediatric home care plans for infants and young children in patients' homes
- 2Deliver tracheostomy/ventilator and infusion care following hospital protocols and family training
- 3Partner with families to teach care skills and ensure smooth hospital-to-home transitions
- 4Coordinate with clinical team to monitor client progress and adjust care plans as needed
- 5Document care, outcomes, and family education in the patient record
- 6Support multidisciplinary team during initial weeks of home care and provide ongoing guidance to caregivers
- 7Deliver compassionate, patient-family centered care in a home setting
- 8Maintain compliance with all regulatory standards and BAYADA policies
- 9Educate families on safety, equipment use, and emergency plans
- 10Promote BAYADA values and contribute to a positive, collaborative work environment
Requirements5
- 1Have a current license as a Licensed Practical Nurse
- 2Have at least one year of verifiable work experience as an LPN NICU PICU ICU or home care trach/vent experience required
- 3Demonstrated competence in infusion and tracheostomy/ventilator care
- 4Ability to work in a home care setting and adapt to variable schedules
- 5Strong communication and family education skills
