What You'll Do10

  • 1Build and maintain a patient panel through home visits in Philadelphia PA and surrounding areas
  • 2Assess patients health through examinations histories and medical data and develop care plans
  • 3Communicate effectively with office staff and clinical operations across internal platforms
  • 4Provide high quality patient centered care resulting in improved outcomes
  • 5Develop impressions diagnoses and treatment plans with patient and caregiver involvement
  • 6Order diagnostic testing when medically necessary and prescribe medications and interventions
  • 7Foster trust and consult with patients families and caregivers during visits
  • 8Ensure documentation and care coordination align with clinical guidelines and payer requirements
  • 9Collaborate with physicians and the in house team to optimize patient care
  • 10Support through remote monitoring or telemedicine as appropriate

Requirements6

  • 1Board Certified with a valid NP or PA certification in the state of PA
  • 2Strong clinical skills with the ability to accurately treat assess and diagnose
  • 3Positive demeanor and strong people skills
  • 4Ability to problem solve and operate independently
  • 51-2 years of primary care experience with geriatric patients
  • 6Hospice experience preferred but not required
