Overview

Nurse Practitioner or Physician Assistant will deliver primary and palliative care to homebound patients in the DC area on a full-time schedule. You will operate under physician supervision to manage a diverse panel with chronic conditions and limited mobility. The role supports remote monitoring, behavioral health management, and chronic care programs through Ennoble Care's mobile home-based model. You will work with a dedicated clinical team and care for patients, families, and caregivers in a collaborative setting.

What You'll Do5

  • 1Build patient care plans through physical exams, histories, and diagnostic assessments to inform treatment across a DC panel
  • 2Design and implement high quality, patient centered care plans for primary and palliative patients in home settings
  • 3Deliver orders for diagnostic testing and prescribe medications and interventions as medically necessary
  • 4Collaborate with office staff and clinical operations across internal platforms to maintain continuity of care
  • 5Communicate effectively with patients, families, and caregivers to foster trust and adherence to care plans

Requirements5

  • 1Board Certified with a valid NP or PA certification in Washington, DC
  • 2Strong clinical skills to accurately assess, diagnose, and treat diverse patient needs
  • 3Positive demeanor with strong people skills and independent problem solving
  • 41-2 years of primary care experience with geriatric patients
  • 5Hospice experience is preferred but not required
