Overview

Own autonomous patient assessments, diagnoses, and treatment plans at scale within a collaborative healthcare team. This role combines independent practice with multidisciplinary coordination to ensure comprehensive care.

What You'll Do10

  • 1Build comprehensive patient assessments and diagnoses from history, exam, and tests
  • 2Develop and implement individualized treatment plans including pharmacologic and non pharmacologic therapies
  • 3Prescribe and manage pharmacology regimens with monitoring and adjustments as needed
  • 4Utilize Electronic Health Records for documentation, orders, and physician communication
  • 5Educate patients and families promoting health literacy and shared decision making
  • 6Coordinate care with interdisciplinary teams to optimize outcomes and transitions of care
  • 7Demonstrate clinical decision making in complex patient scenarios and adapt care plans
  • 8Collaborate with physicians and other professionals for integrated patient management
  • 9Maintain precise medical documentation reflecting encounters, assessments, and plans
  • 10Engage in effective communication and empathetic patient centered care

Requirements5

  • 1Master`s or Doctorate degree from an accredited NP program
  • 2Current, unrestricted NP license in the state of practice
  • 3Board certification as a Nurse Practitioner (e.g., ANCC, AANP)
  • 4Two (2) or more years of clinical NP experience
  • 5Experience in acute care or specialized outpatient settings preferred
