Nurse Practitioner, Primary & Palliative Care PA
Overview
You will own primary and palliative care for homebound patients across Montgomery County, PA, managing a panel of 100+ patients with chronic conditions. You work autonomously under physician supervision, supported by our in-house clinical team. This role combines mobile care with the resources of a national provider, letting you focus on patient outcomes. You will attend quarterly medical staff meetings and shape care plans for a diverse patient population.
What You'll Do7
- 1Provide high quality primary and palliative care to patients in and around Montgomery County, PA, during home visits.
- 2Assess patient health by performing physical exams, obtaining and studying medical histories, and updating records.
- 3Create and implement care plans, including diagnoses, treatments, and medication prescriptions.
- 4Order diagnostic testing and prescribe medically necessary interventions.
- 5Communicate with office staff and clinical operations across internal platforms like EMR and Telehealth.
- 6Foster trust with patients, families, and caregivers, consulting them during visits.
- 7Attend quarterly medical staff meetings and contribute to care coordination.
Requirements6
- 1Board certified Nurse Practitioner with a valid NP certification in Pennsylvania.
- 21-2 years primary care experience with geriatric patients, hospice preferred but not required.
- 3Strong clinical skills to assess, diagnose, and treat chronic conditions.
- 4Ability to operate independently and problem-solve in home settings.
- 5Positive demeanor and strong people skills for patient interaction.
- 6Valid DEA license and Pennsylvania prescriptive authority.
