Overview
Mulberry Health seeks a dedicated Primary Care Nurse Practitioner to deliver in-home visits across Phoenix Metropolitan Area. This role offers independence while supporting a growing team of over 20 providers. Candidates will manage chronic conditions and coordinate care with specialists.
What You'll Do10
- 1Conduct in-home visits within assigned service territory
- 2Provide comprehensive primary care services
- 3Diagnose and manage acute and chronic medical conditions
- 4Perform annual wellness visits and preventive care assessments
- 5Coordinate care with specialists and hospice teams
- 6Document visits using eClinicalWorks
- 7Educate patients and caregivers on health maintenance
- 8Participate in interdisciplinary care planning
- 9Support care transitions and medication management
- 10Deliver exceptional patient-centered care to seniors and complex patients
Requirements10
- 1Certified Nurse Practitioner (FNP-C, AGNP-C, or equivalent)
- 2Active Arizona Nurse Practitioner License
- 3Valid Arizona Driver License
- 4Reliable transportation
- 5Strong communication and clinical documentation skills
- 6Ability to work independently and manage flexible schedule
- 7Experience with value-based care models
- 8Home-Based Primary Care experience
- 9Geriatric experience
- 10Assisted Living or Group Home experience
