What You'll Do6
- 1Perform comprehensive history and physical exams, routine follow-ups, and urgent evaluations for patients in nursing homes and home settings.
- 2Diagnose and manage acute and chronic conditions, ordering and interpreting diagnostic tests and labs using EHR systems.
- 3Develop and manage individualized treatment plans, coordinating care with primary care physicians, specialists, and nursing staff.
- 4Educate patients and families on disease management, medications, and preventive care to reduce hospital readmissions.
- 5Document all patient encounters accurately and timely in the EHR, ensuring compliance with regulations.
- 6Drive quality improvement and population health initiatives, analyzing patient outcomes to enhance care delivery.
Requirements5
- 1MSN or DNP from an accredited Nurse Practitioner program.
- 2Active or eligible New York State NP license and national certification (FNP, AGNP, or GNP preferred).
- 30-2+ years in nursing home, house call, geriatrics, or primary care; new graduates with strong clinical training are encouraged to apply.
- 4Proficiency with EHR platforms for documentation and care coordination.
- 5Ability to travel to patient locations within Manhattan and the Bronx, with reliable transportation.
