Nurse Practitioner - In-Home & Telehealth Care
Overview
You will own patient care delivery for in-home and telehealth visits, serving diverse communities across Houston. This hybrid role combines face-to-face visits with virtual appointments, enabling deep patient relationships and personalized treatment. You will join ACE Physicians, a sister company of Varco Healthcare, and help establish our presence in a new region. Early providers gain autonomy and a direct impact on care models, backed by an established organization committed to accessible healthcare.
What You'll Do7
- 1Perform comprehensive health assessments during in-home visits and telehealth appointments, focusing on acute and chronic conditions.
- 2Develop individualized treatment plans using eClinicalWorks or similar EMR, ensuring continuity across settings.
- 3Educate patients and families on disease prevention and wellness, tailoring advice to individual needs.
- 4Collaborate with physicians and interdisciplinary teams to coordinate care for complex cases.
- 5Conduct minor procedures such as wound care during home visits when clinically appropriate.
- 6Document every encounter accurately and promptly, meeting compliance standards.
- 7Advocate for equitable care, including for patients with disabilities, ensuring respectful and inclusive treatment.
Requirements6
- 1Active, unrestricted Nurse Practitioner license.
- 25+ years in geriatrics, internal medicine, or primary care preferred.
- 33-5 years in home health or home-based care strongly preferred.
- 4Experience caring for patients with disabilities is highly desirable.
- 5Comfortable delivering care in both in-home and telehealth settings.
- 6Excellent communication and a collaborative, team-oriented mindset.
