Remote Urgent Care Nurse Practitioner - Medicare Telehealth
Overview
Belle seeks a compassionate Nurse Practitioner to provide remote telehealth support for Medicare beneficiaries in our Chronic Care Management program. You will assess urgent non-emergent clinical concerns and collaborate with nurses and care teams to prevent emergency department visits. What You'll Do9
- 1Conduct telehealth evaluations for Medicare beneficiaries experiencing urgent non-emergent clinical concerns
- 2Evaluate symptoms review medical histories and determine appropriate interventions
- 3Determine when escalation to primary care provider emergency department or 911 is clinically indicated
- 4Document all encounters accurately and timely in the electronic medical record
- 5Assist in reducing avoidable emergency department visits and hospitalizations through proactive clinical intervention
- 6Provide patient education on disease management medications lifestyle modifications and symptom monitoring
- 7Promote medication adherence and self-management of chronic conditions
- 8Provide clinical guidance and education to nursing staff on appropriate escalation pathways
- 9Participate in clinical case reviews and quality improvement initiatives
Requirements9
- 1Active unrestricted Nurse Practitioner license Georgia preferred
- 2National certification appropriate to specialty
- 3Minimum of 3 years clinical practice experience preferably in primary care internal medicine family medicine geriatrics urgent care or chronic disease management
- 4Experience managing hypertension and common chronic conditions including diabetes heart failure COPD and chronic kidney disease
- 5Prior telehealth or virtual care experience
- 6Strong clinical assessment and decision-making skills
- 7Excellent verbal communication and patient education abilities
- 8Familiarity with eClinicalWorks eCW or similar EMR systems
- 9Competitive compensation based on experience
