Overview
Telehealth Nurse role supporting members with chronic conditions through remote patient monitoring. This position offers full-time hours Monday through Friday 8:30am-5pm local time with benefits and remote work options. The role focuses on delivering personalized care to enhance health outcomes.
What You'll Do11
- 1Perform regular telehealth calls to a dedicated panel of members
- 2Provide medical diet and wellness education to members
- 3Ensure accurate documentation and maintain good member nurse rapport
- 4Drive clinical outcomes through member engagement and coordination
- 5Escalate members to provider team as needed
- 6Offer device support and troubleshooting professionally
- 7Maintain rapport with members nursing team and providers
- 8Foster a culture of kindness compassion and mutual respect
- 9Assist with special projects and other assignments
- 10Conduct high volume calls with focus on quality
- 11Communicate effectively across diverse member populations
Requirements11
- 1Current active unrestricted LPN RN multi-state compact license in a nursing compact state
- 2Minimum 2 years hands-on nursing experience with telehealth and chronic care management preferred
- 3EHR experience specifically AthenaHealth is a plus
- 4Fluency in English and Spanish
- 5Comfortable using computers and multiple applications during calls
- 6Macbook experience is a plus
- 7Reliable high-speed internet connection
- 8Strong time management skills and self-motivation
- 9Quick learning ability in fast paced environment
- 10Professional communication style meeting CopilotIQ core values
- 11Ability to work independently and collaborate effectively
