What You'll Do8
- 1Deliver LPN nursing care to patients in their homes, including wound care, medication administration, and vital sign monitoring.
- 2Assess patient conditions and report changes to supervising RN or physician.
- 3Educate patients and families on care plans and disease management.
- 4Document patient encounters in EHR systems and submit timely records.
- 5Coordinate with Medside Healthcare team to ensure continuity of care across visits.
- 6Drive up to 70 miles per day to reach patient homes and adjust schedule based on patient needs.
- 7Maintain LPN licensure and follow state regulations for home health practice.
- 8Participate in care conferences and communicate with interdisciplinary team.
Requirements7
- 1Current LPN license in Georgia or compact state, with no sanctions.
- 21+ year of clinical experience in a healthcare setting.
- 3Reliable transportation and ability to travel within a 70-mile radius.
- 4CPR BLS certification (preferred).
- 5Home health or Medicare experience (preferred).
- 6Comfortable working independently and making nursing judgments.
- 7Strong communication and documentation skills.
Salary Insight
$83 - $92k per year
