What You'll Do6
- 1Provide skilled nursing services to adult and geriatric patients in their homes following individualized care plans
- 2Monitor assess and document patient progress updating care plans as needed
- 3Administer medications and perform prescribed treatments according to state regulations
- 4Educate patients and families on ongoing care needs disease prevention and health management
- 5Maintain accurate timely records using HomeCare HomeBase (HCHB) EMR
- 6Collaborate with RNs therapists and other healthcare professionals to ensure comprehensive care solutions
Requirements6
- 1Active LPN license required
- 2At least one year of recent home health experience
- 3Proficiency with HomeCare HomeBase (HCHB) electronic medical records
- 4Excellent clinical assessment communication and documentation abilities
- 5Reliable transportation for patient visits
- 6Ability to work independently and efficiently manage a caseload
