The Patient Care & Technology Coordinator supports high-risk patients using AI-powered predictive analytics and remote patient monitoring. This role serves as a key liaison between patients, providers, and caregivers while ensuring effective communication and timely intervention. What You'll Do11
- 1Build and design remote patient monitoring programs to track patient vitals and alerts
- 2Conduct in-home visits to install and configure RPM devices and educate patients
- 3Perform mobile phlebotomy to collect lab specimens following clinical protocols
- 4Review alerts and coordinate care among providers and specialists
- 5Document patient encounters and ensure regulatory compliance
- 6Coordinate telemedicine visits and prepare clinical updates for providers
- 7Triage patient concerns and escalate urgent issues to physicians
- 8Maintain accurate records and process medical requests
- 9Utilize EMRs and monitoring dashboards to track patient status
- 10Support transitions of care and manage home health populations
- 11Advocate for patient needs and resolve care coordination barriers
Requirements8
- 1Bilingual English Spanish or English Haitian Creole required
- 2Associate or bachelor’s degree in nursing
- 3Minimum three years of healthcare experience in clinical settings
- 4Strong understanding of medical terminology and chronic disease management
- 5Proficiency with EMRs telehealth platforms and digital monitoring tools
- 6Experience with high-risk patient populations including cardiovascular and pulmonary cases
- 7Certifications in case management or care coordination preferred
- 8Ability to work independently in a fast-paced environment
