Clinical Care Nurse - Florida
Overview

What You'll Do9
- 1Analyze clinical data from Athena EMR and DataHub to identify gaps aligned with Medicare Advantage Stars and HEDIS measures
- 2Coordinate post-discharge follow-up for recent inpatients using phone telehealth and in-clinic visits
- 3Conduct medication reconciliation and symptom monitoring assessments during outreach
- 4Collaborate with providers and interdisciplinary teams to prevent readmissions and avoidable ED visits
- 5Document all interactions and outcomes in compliance with HIPAA regulations
- 6Participate in quality improvement projects and attend center huddles to align on clinical priorities
- 7Facilitate wellness events and manage clinic operations through provider partnerships
- 8Lead evidence-based interventions to optimize workflows and patient engagement
- 9Maintain accurate medical records and adhere to organizational policies
Requirements7
- 1Associate's or Bachelor's degree in Nursing with active Florida RN license
- 23+ years' experience in transitions of care quality improvement or population health management
- 3Proficiency with electronic health records and data analytics tools
- 4Basic Life Support certification and willingness to maintain current training
- 5Strong communication skills with motivational interviewing capabilities
- 6Bilingual English and Spanish fluency preferred
- 7Commitment to health equity and culturally competent care delivery
