What You'll Do5
- 1Lead patient care conferences with multidisciplinary teams to drive timely transitions and escalate care delays to leadership
- 2Assess readmitted patients for perceived reasons and advocate for patient empowerment in decision making
- 3Document discharge planning evaluations and progressions according to standard work practices
- 4Organize care conferences to mobilize family resources and educate patients on emotional social and financial impacts of illness
- 5Own and scale ETL pipeline initiatives using Spark and Airflow to improve data accuracy
Requirements9
- 15+ years building ETL pipelines with Spark and Airflow
- 22+ years medical hospital nursing experience
- 3Prior Care Management experience preferred
- 4Registered Nurse license required
- 5AASN certification preferred
- 6Florida Level 2 background screening clearance
- 7Associate's degree in nursing required
- 8Bachelor's degree in nursing preferred
- 9Strong communication skills to coordinate multidisciplinary efforts
