Nurse Practitioner or Physician Assistant provide primary and palliative care to a diverse patient panel in Baltimore County. What You'll Do10

  • 1Build strong patient relationships through in person visits in the home and clinic
  • 2Assess health, obtain histories, and develop care plans for chronic conditions and end of life needs
  • 3Communicate with office staff and clinical operations across internal platforms
  • 4Develop impressions, diagnoses, and treatment plans for patients
  • 5Order diagnostic testing as medically necessary and prescribe medications and interventions
  • 6Foster trust with patients, families, and caregivers during visits
  • 7Provide high quality, patient centered care to a defined panel of patients
  • 8Collaborate with in house clinical team to support orders and escalation
  • 9Maintain documentation and comply with MD state regulations
  • 10Participate in continuous improvement and program growth across regions

Requirements6

  • 1Board Certified with a valid NP or PA certification in Maryland
  • 2Strong clinical skills to accurately diagnose and treat in primary care and geriatric populations
  • 3Positive demeanor and strong people skills
  • 4Ability to problem solve and operate independently
  • 51-2 years of primary care experience with geriatric patients
  • 6Hospice experience preferred but not required
