Palliative Care Advanced Practice Clinician, Oakland CA & Oncology Collaboration
Overview
Bold APRN or PA with post graduate palliative care, hospice, oncology, or geriatrics experience. Own care delivery across clinic, home, and facilities with a multidisciplinary team. Collaborate with MDs, RNs, social workers, and chaplains; participate in embedded oncology practice at SMSHCC. Enjoy regular hours, no night shifts or weekend call, and opportunities for leadership and advancement.
What You'll Do5
- 1Build and guide treatment plans aligned with patient and family values across clinic, home, and facility settings
- 2Design and deliver palliative care interventions with an integrated team including MD, RN, and social work
- 3Ship patient care through telemedicine and in-person visits to improve symptom management and quality of life
- 4Own coordination with oncology partners at SMSHCC to integrate palliative goals into cancer treatment plans
- 5Lead care discussions that reflect patient-centered goals within a multidisciplinary framework
Requirements5
- 1For NP: credentialed as ACHPN and current APRN license
- 2For PA: PA certificate in Palliative and Serious Illness Care and NCCPA certification
- 312+ months of post-graduate clinical experience in hospice, palliative care, oncology and/or geriatrics
- 4Ability to work in a community-based palliative care setting across multiple environments
- 5Eligibility for sabbatical and schedule flexibility considered for this role
Salary Insight
$184 - $226k per year
