What You'll Do11
- 1Build and perform wound care procedures during patient visits to support a comprehensive plan of care
- 2Notate findings and treatment course in patient chart, including follow-up notes
- 3Define and document patient diagnoses and tailor care plans accordingly
- 4Provide patient education and management to ensure understanding and adherence
- 5Apply topical wound care and other wound management products
- 6Debride wounds by various methods safely during the patient visit
- 7Order and perform tests as needed to inform treatment decisions
- 8Ensure patient satisfaction through clear communication and responsiveness
- 9Maintain accurate documentation and coordinate care with agencies and team members
- 10Drive to patient homes within the San Jose area to deliver mobile wound care
- 11Notate and track outcomes to monitor healing progress
Requirements8
- 1Current CA license as NP or PA
- 2CPR Certification
- 3Valid driver’s license and minimum state-required liability auto insurance
- 4Excellent communication and interpersonal skills
- 5Willing to drive to patient homes for wound care and local travel
- 6No prior wound care experience required as training is provided
- 7Must be able to work full-time, part-time, or contract arrangements
- 8Ability to manage multiple patient visits per day and maintain chart accuracy
