Overview

Personic Health seeks a Nurse Practitioner to own clinical care for a mobile wound care practice serving Philadelphia, PA. You will perform advanced procedures at patient bedsides in assisted living and residential settings. Working with physician leadership and a multidisciplinary team, you will manage acute and chronic wounds using cellular tissue products. This role offers comprehensive wound care training and autonomy in a physician-founded organization.

What You'll Do8

  • 1Perform bedside wound care procedures including debridement, biopsies, suturing, and injections using scalpels and cellular tissue products.
  • 2Assess and diagnose acute and chronic conditions, then develop and implement treatment plans for patients in their homes.
  • 3Collaborate with physicians and healthcare professionals to coordinate comprehensive care across facilities.
  • 4Conduct patient and family education on treatment plans, preventive care, and lifestyle adjustments.
  • 5Maintain precise medical records using electronic health record (EHR) systems after each visit.
  • 6Participate in quality improvement initiatives and adhere to clinical practice guidelines for optimal outcomes.
  • 7Provide leadership and mentorship to nursing staff and supporting healthcare providers in the field.
  • 8Stay current on advances in wound care technology and procedures to improve patient outcomes.

Requirements9

  • 1Master's or Doctorate degree in Nursing from an accredited institution.
  • 2Current Nurse Practitioner licensure in Pennsylvania.
  • 3Certification as a Nurse Practitioner (e.g., ANCC or AANP).
  • 41+ year experience as a Nurse Practitioner in a procedural heavy setting, but new graduates with relevant clinical rotations are considered.
  • 5Demonstrated expertise in wound care management, including assessment, treatment, and monitoring of acute and chronic wounds.
  • 6Proficiency with scalpels for procedures and wound management in clinical settings.
  • 7Skill in performing a variety of medical procedures with high accuracy and confidence.
  • 8Strong critical thinking and problem-solving abilities for complex patient scenarios.
  • 9Commitment to compassionate, patient-centered care and ongoing education.
