What You'll Do10
- 1Build and execute wound care plans during home visits in patients' homes or SNFs to improve healing outcomes
- 2Diagnose and document patient conditions while delivering debridement and topical wound therapies
- 3Provide patient education and management to ensure understanding and adherence to the treatment plan
- 4Perform tests and procedures relevant to wound assessment and treatment
- 5Notate findings, treatment course, and follow-up plans in the patient chart
- 6Ensure patient satisfaction and maintain high standards of care and communication with agencies
- 7Apply wound care products and manage debridement using approved methods
- 8Coordinate care across the local region to cover patient load effectively
- 9Drive to multiple patient sites within Denver and surrounding areas as needed
- 10Maintain safety and compliance with licensure, driving requirements, and care protocols
Requirements10
- 1Current CO NP or PA license
- 2CPR certification
- 3Excellent communication and customer service skills
- 4Valid driver's license and minimum state-required liability auto insurance
- 5Willingness to drive to patient homes and travel locally
- 6No prior wound care experience required as training is provided
- 7Ability to work onsite in patient homes and SNFs
- 8Strong documentation habits and attention to detail
