What You'll Do10

  • 1Assess patient and family needs in crisis, identifying appropriate resources across the Emergency Department, Trauma, and hospital units.
  • 2Conduct behavioral health assessments and coordinate admissions, transfers, and discharge plans with physicians and interdisciplinary team members.
  • 3Develop interventions for transitions from emergency acute care to the next level of care throughout the continuum.
  • 4Maintain a working knowledge of community resources and assist patients and families in obtaining needed support services.
  • 5Collect data and monitor quality of practice based on access, quality, and identified departmental outcome indicators.
  • 6Build positive relationships with internal and external customers as a representative of the health system.
  • 7Participate as a team member of the clinical interdisciplinary team to ensure appropriate care, support, and resources.
  • 8Float throughout the District as needed to provide consistent social work coverage.
  • 9Follow legal guidelines in mandatory reporting and comply with Palomar Health policies, procedures, and applicable laws.
  • 10Carry out the mission, vision, and quality commitment of Palomar Health.

Requirements7

  • 1Master's Degree in social work.
  • 22-3 years of related medical social work experience (preferred 3-5 years).
  • 3Speak and read English at a level sufficient to perform essential job functions.
  • 4Knowledge of standard office equipment and computer software including MS Windows, Excel, Access, Word, PowerPoint, internet, and e-mail.
  • 5Proficient keyboarding and mouse skills, including using drop-down menus, scroll bars, and opening folders within first two weeks of employment.
  • 6Ability to perform other duties as assigned.
  • 7Carry out Palomar Health rules, policies, and standards.
