What You'll Do5
- 1Render clinical psychosocial services to patients and families of all ages, neonate to geriatric, in an inpatient setting.
- 2Assess patients' psychosocial needs and coordinate referrals to appropriate facilities or community agencies.
- 3Provide crisis intervention and bereavement counseling as part of comprehensive patient care.
- 4Collaborate with the multidisciplinary team to support discharge planning, care coordination, and patient advocacy.
- 5Document assessments, plans, and interventions in the medical record per policy and standards.
