Requirements9
- 15+ years social work experience with mastery of chronic disease impact on families
- 2Masters in Social Work degree from CSWE-accredited program
- 3Certification as LCSW preferred with additional social service experience
- 4Proven ability to perform complex benefit coordination and care planning
- 5Strong multitasking skills for high-volume clinical environments
- 6Experience with electronic health record systems and automated clinical tools
- 7Excellent written documentation and interpersonal communication abilities
