Overview

As an LCSW in Women's Health at Sutter Health, you will own biopsychosocial assessments, crisis intervention, and short-term counseling for patients across all ages in any care setting. You will partner with a multidisciplinary team to drive discharge planning and transitions of care, while providing psychoeducation and facilitating support groups. Your role includes leading goals of care conversations and serving as a liaison to community programs. What You'll Do8

  • 1Conduct biopsychosocial assessments and provide crisis intervention in acute and outpatient settings within the first 30 days.
  • 2Deliver short-term counseling and grief counseling to patients and families adjusting to illness or life transitions.
  • 3Facilitate support groups and psychoeducation sessions for patients with chronic conditions.
  • 4Collaborate with the interdisciplinary team to create discharge plans and coordinate transitions of care.
  • 5Lead goals of care conversations and provide end-of-life support using palliative care principles.
  • 6Advocate for patients and families, linking them to community resources and at-risk population support.
  • 7Document all interactions in EPIC and maintain compliance with Tarasoff and reporting requirements.
  • 8Educate staff on psychosocial aspects of care and participate in committees to improve patient outcomes.

Requirements10

  • 1Master's degree in Social Work or a related field.
  • 2Current LCSW license in the state of California.
  • 32+ years of recent clinical social work experience in a healthcare setting.
  • 4Proficiency in biopsychosocial assessments and clinical interventions including crisis intervention and motivational interviewing.
  • 5Knowledge of transitions of care and community resources for discharge planning.
  • 6Familiarity with post-acute levels of care and resource needs for safe discharge.
  • 7Understanding of child and elder abuse reporting requirements and confidentiality laws.
  • 8Ability to assess suicidal behavior and implement safety plans.
  • 9Skill in evaluating substance use and developing treatment plans.
  • 10Demonstrated collaboration skills and ability to work effectively in a fast-paced environment.
