Overview
Own daily social services operations and mentor a team of social workers while setting standards for compassionate care. Shape patient experiences in home settings and drive continuous improvement. What You'll Do9
- 1Lead daily operations of our social services team including policies procedures and care coordination with clinical providers
- 2Recruit onboard and mentor social workers providing clinical supervision and supporting professional growth
- 3Oversee assessment planning and evaluation of social services care ensuring documentation meets state federal and ACHC standards
- 4Partner with Executive Director on annual department budget and resource planning
- 5Build knowledge of community resources connecting patients to appropriate support services
- 6Plan in-service training and continuing education to maintain team skills
- 7Contribute to quality improvement initiatives identifying new ways to serve patients
- 8Perform other duties as required for patient services
- 9Maintain self-directed organized flexible approach taking initiative and collaborating with others
Requirements8
- 1Master of Social Work MSW from an accredited program with an active California LCSW license
- 2At least 3 years management experience preferably in home care
- 3Strong knowledge of healthcare regulations ACHC standards and business operations
- 4Excellent communication skills building trust with patients families staff and community
- 5Self-directed flexible approach taking initiative and working well with others
- 6Flexible schedule convertible to full-time based on performance
