Intake Specialist, Social Services & Housing Support
Overview
Own intake and assessment processes for prospective members at Justa Center, guiding pathways to permanent housing at scale. Work with the care coordination team to develop individualized strategies and connect members to community resources. What You'll Do8
- 1Build and conduct up to four in-depth intakes per day, 60-90 minutes each, to assess needs and empower members
- 2Inform new members about Justa Center services, resources, and partnerships
- 3Collaborate with care coordination to establish milestones based on assessment conclusions
- 4Connect with members to access community resources including Social Security, healthcare coverage, IDs, legal aid, domestic violence support, and substance abuse programs
- 5Maintain an active resource list with contacts for DES, Area Agency on Aging, CASS, SvDP, CBI, UMOM, faith-based orgs, AHCCCS, AZDVS, AZDHS, and Homeless ID Project
- 6Enter and manage member data in HMIS and CaseWorthy databases
- 7Engage with care coordination staff and Director of Operations to review progress and ensure continuity of care
- 8Perform other duties as requested
Requirements6
- 1Bachelor’s or associate degree in social work, public health, or human services required
- 2Three years of experience in counselling, social services, and/or elder care
- 3Knowledge of services for the homeless senior population in Maricopa County
- 4Strong skills in HMIS, Excel, and data management
- 5Bilingual proficiency is a plus
- 6Valid AZ driver’s license and clean driving record
