Overview
Lead social services program establishing action plans by exploring options and setting goals with residents and families. Assist residents in achieving self-care independence and well-being. Provide medical social services ensuring highest practicable physical mental and psychosocial wellbeing. Offer emotional support addressing coping with disabilities fears and grief. Support healthcare decisions personal shopping inventory management and resource acquisition. Coordinate responses to missing belongings secure prosthetics and assistive devices. Build relationships with departments and community agencies. Document resident status and schedule outpatient services. Provide in-service training and counseling.

- 1Establish course of action by exploring options and setting goals with resident and families
- 2Assist residents in achieving highest practicable level of self-care independence and wellbeing
- 3Provide medically related social services for highest practicable physical mental and psychosocial wellbeing
- 4Evaluate social and family information psychological emotional needs to assess social services needs and develop care plans
- 5Offer emotional support addressing coping with physical disabilities fears related to health conditions and grief
- 6Assist residents with healthcare decisions personal shopping inventory tracking and resource acquisition
- 7Coordinate response to missing lost or stolen belongings secure prosthetics and assistive devices
- 8Develop and maintain relationships with departments and community agencies
- 9Document resident social service status
- 10Assist in making outpatient appointments and schedule ancillary patient services
- 11Assist in discharge planning with appropriate agencies equipment and referrals
- 12Coordinate with interdisciplinary team
- 13Provide in-service training to staff as requested
- 14Counselling residents and family members
Requirements6
- 1Bachelor’s Degree in Social Work or Human Services with 2 years supervised healthcare social work experience
- 2MSW preferred
- 3Knowledge and experience with PCC
- 4Physical demands standing walking frequently reaching pushing pulling talking hearing frequently lifting up to 25 pounds
- 5Ability to interpret written oral diagram schedule instructions
- 6Certificates licenses registrations
