Overview

Own the after hours hospice care experience for a community in Austin, delivering compassionate patient-centered nursing at scale. You will join a collaborative interdisciplinary team to execute plans of care, provide education, and ensure seamless transitions. This role offers a full-time schedule with a 7 on 7 off pattern and competitive compensation. You’ll work onsite with access to benefits and professional development opportunities.

What You'll Do6

  • 1Lead comprehensive nursing assessments and collaborate with the Interdisciplinary Group to develop and evaluate care plans
  • 2Maintain timely documentation and coordinate resources for complex or high risk cases
  • 3Provide education and emotional support to patients and caregivers based on individual needs
  • 4Participate in on-call rotations admissions and care transitions
  • 5Coordinate care management including scheduling medication management and supervision of licensed staff when acting as Care Manager
  • 6Respond to triage calls perform visits and assessments in on-call capacity and support IDG meetings and family conferences

Requirements10

  • 1Current RN licensure in the applicable state
  • 2Associate degree preferred
  • 3CPR certification recommended or required for direct patient care where applicable
  • 4Experience in hospice home health medical surgical geriatrics or pediatrics preferred
  • 5Willingness to participate in on-call rotation and maintain reliable transportation and valid driver’s license
  • 6Proficiency in Microsoft Office Suite and EMR systems
  • 7Ability to collaborate effectively within an interdisciplinary team
  • 8Ability to perform physical tasks including lifting patients and moving supplies as required
  • 9Strong communication and organizational skills
  • 10Demonstrated commitment to compassionate patient care
