On-Call Registered Nurse - Hospice On-Call
Overview
The On-Call Registered Nurse provides after-hours and weekend clinical support to hospice patients and families in the Denver metro area. This role involves a 7-days-on / 7-days-off rotation serving as the primary clinical contact during non-business hours. Candidates must be compassionate clinically skilled nurses comfortable working independently in home or facility settings.
What You'll Do11
- 1Respond to after-hours patient and family calls; triage symptoms and provide clinical guidance reassurance and education
- 2Conduct urgent in-home or facility-based patient visits as clinically indicated
- 3Assess changes in patient condition and implement or adjust the plan of care within hospice guidelines
- 4Administer or supervise medications and treatments in accordance with physician orders
- 5Provide comfort-focused symptom management including pain dyspnea anxiety and other end-of-life symptoms
- 6Coordinate with the on-call physician for new or updated orders as needed
- 7Communicate significant changes in patient status to the attending physician Director of Nursing and appropriate team members
- 8Provide emotional support and education to patients and families during the on-call period
- 9Complete accurate timely clinical documentation in the EMR according to CMS and organizational standards
- 10Coordinate admissions and pronouncements of death as needed during the on-call period
- 11Ensure continuity of care by providing detailed hand-off communication at the end of each rotation
Requirements11
- 1Current Registered Nurse licensure in the State of Colorado in good standing
- 2Associates Degree in Nursing required Bachelor of Science in Nursing preferred
- 3Minimum one year of RN experience with hospice palliative care home health or ICU/med-surg experience strongly preferred
- 4Prior on-call or independent practice experience preferred
- 5Strong clinical assessment skills and ability to make independent nursing judgments
- 6Proficiency in symptom management pain control and end-of-life care
- 7Excellent communication skills with patients families and interdisciplinary team members
- 8Ability to navigate sensitive emotionally charged situations with compassion and professionalism
- 9Comfortable working independently in various home and facility environments
- 10Proficiency with electronic medical records EMR hospice tools experience a plus
- 11Valid Colorado driver’s license reliable personal transportation auto insurance meeting company minimum requirements ability to pass background check and drug screening CPR/BLS certification required
