Home Health Registered Nurse, Geriatric Care in Austin
Overview
RN with home health experience provides compassionate care to a geriatric population in Austin TX. What You'll Do10
- 1Build comprehensive assessments of physical functional psychosocial and environmental needs and document findings
- 2Design and implement the plan of care ensuring quality and continuity of care and revise as needed
- 3Communicate effectively with patient family team members and other health care professionals
- 4Apply infection control and standard precautions during patient care activities
- 5Provide clinical directions to Hospice Aide and LPN/LVN to ensure quality care
- 6Prepare clinical progress notes showing progress toward goals
- 7Monitor cases for third party payor requirements and ensure documentation is complete
- 8Educate patient and family about disease process self-care end of life care nutrition and needs
- 9Prioritize needs within the IDG and coordinate services based on acuity and needs
- 10Provide training to other staff as needed and assess caregiver ability to meet patient needs
Requirements8
- 1Current RN license and in good standing with the state nursing board
- 2Experience in home health or related healthcare setting preferred
- 3Knowledge or experience with Homecare Homebase EMR would be a plus
- 4Strong clinical and assessment skills
- 5Excellent communication and interpersonal abilities
- 6Ability to work independently and manage time effectively
- 7Compassionate and patient-centered approach
- 8Ability to work onsite in Austin TX
Salary Insight
