Overview
You own clinical care for seniors across DFW, conducting in-home assessments and training caregivers to deliver RN-level care. You work with seniors facing Alzheimer's, dementia, hospice, and general geriatric needs, supported by a team that values Dignity, Discretion, and Patience. This contract role stands out for its focus on converting assessments into active clients while maintaining high clinical standards.
What You'll Do10
- 1Conduct comprehensive in-home nursing assessments for new and existing clients across the DFW area.
- 2Convert qualified prospects into active home care clients through education, relationship building, and exceptional service.
- 3Develop, implement, and update individualized plans of care for geriatric clients.
- 4Delegate nursing tasks and provide hands-on training to caregivers in line with Texas state regulations.
- 5Perform nursing procedures including medication management, clinical assessments, and other services within the RN scope of practice.
- 6Monitor client progress and communicate changes in condition to physicians, families, and the care team.
- 7Complete accurate clinical documentation, assessments, and required reports.
- 8Serve as a clinical resource for caregivers, providing education, coaching, and ongoing support.
- 9Collaborate with office staff to ensure continuity of care and exceptional client service.
- 10Promote a safe, compliant, and high-quality care environment for every client.
Requirements6
- 1Hold a full, active license as a Registered Nurse.
- 22+ years of related work experience in a geriatric healthcare setting.
- 3Demonstrate flexibility for assessments and training scheduling.
- 4Show effective verbal and written communication skills.
- 5Work independently with initiative, problem-solving ability, and adaptability.
- 6Perform clinical skills including documentation of care, medication administration, infection control precautions, assessing patient educational needs, physical assessment, obtaining vital signs, and care coordination.
