Hospice Nurse Practitioner, Palliative Care in Austin TX
Overview
You own clinical oversight for hospice patients across Austin, TX, guiding care for terminal illnesses in partnership with physicians and the Interdisciplinary Group. You lead the IDG in developing and executing individualized care plans, and you educate families on symptom management and end-of-life options. This role weaves patient visits, face-to-face encounters, and on-call rotations into a single flow, with CPC Card or Family Nurse Practitioner certification as your entry point. You directly shape quality outcomes for patients facing their final months, with autonomy to adjust care in real time.
What You'll Do10
- 1Lead the IDG to evaluate and treat terminal patients, setting the palliative direction for each case.
- 2Design individualized care plans with patients and families, adjusting as conditions change.
- 3Teach patients and families about disease progression, symptom control, and self-care at end of life.
- 4Conduct patient visits per the care plan, documenting each encounter in the EMR within 24 hours.
- 5Coordinate with attending and consulting physicians on palliative treatment, channeling input through the hospice physician.
- 6Guide hospice staff on clinical issues, from symptom management to ethical dilemmas.
- 7Drive IDG meetings with clear updates and follow-through on action items.
- 8Complete face-to-face patient encounters before the 3rd benefit period and for every recertification, reporting findings to the hospice physician immediately.
- 9Keep infection control and PHI compliance tight during every visit and interaction.
- 10Rotate on-call coverage, responding to urgent patient and family needs within the hour.
Requirements8
- 12+ years as a Family Nurse Practitioner in hospice or palliative care.
- 2Active Certified Family Nurse Practitioner license in Texas.
- 3BLS Certification current and maintained.
- 4Experience collaborating with multidisciplinary teams in end-of-life settings.
- 5Skilled in patient education on symptom management and advance care planning.
- 6Proficiency with EMR systems for accurate, timely documentation.
- 7Ability to commute to Austin, TX 78729 and relocate before start date.
- 8Willingness to participate in on-call rotation, including weekends.
