Overview

Social services professionals will own high-impact housing stabilization for individuals and families experiencing homelessness across Salt Lake County. The HiFI Case Manager provides person-centered, trauma informed support to secure and maintain housing, build community connections, and access specialized services. You will collaborate with partner agencies and field teams to coordinate care in person at resource centers and shelters. This role is in person with travel between sites and focuses on compassionate, non judgmental engagement and harm reduction.

What You'll Do10

  • 1Build direct case management services including crisis intervention assessments goal setting and connection to resources to help participants achieve housing stability
  • 2Design outreach in homes community centers and outdoor settings to reach participants where they are
  • 3Maintain accurate timely records of services and documentation to ensure compliance
  • 4Lead participation in weekly clinical case staffing and supervision to support progress and professional growth
  • 5Ship transportation logistics using agency vehicles to accompany participants to appointments housing viewings and related activities
  • 6Drive collaboration with community partners to coordinate care and share updates for seamless support
  • 7Own participant connections to healthcare mental health substance use services and employment opportunities
  • 8Scale participant engagement through open non judgmental conversations about goals using a harm reduction approach
  • 9Drive participation in mandatory trainings and emergency drills to uphold safety and care standards
  • 10Ensure adherence to safety security and sanitation protocols across environments

Requirements10

  • 1Lived work or volunteer experience in social services or related field is preferred and a degree in a related field is required
  • 2Must be authorized to work in the U.S. without sponsorship
  • 3This is an in person role requiring travel between resource centers and shelters
  • 4Eligible to drive a vehicle and possess a valid driver’s license
  • 5Ability to lift up to 25 pounds and to sit stand and move for extended periods
  • 6Strong interpersonal and communication skills with diverse populations
  • 7Comfort with trauma informed and harm reduction approaches
  • 8Experience collaborating with multiple partner agencies and community resources
  • 9Demonstrated reliability dependability and ability to work with minimal supervision
  • 10Commitment to honoring diverse backgrounds and promoting inclusive practice
