Overview
Mastery of behavioral health case management with leadership of an Assertive Community Treatment ACT team at Horizon House. What You'll Do5
- 1Lead and supervise ACT team operations ensuring regulatory and agency compliance.
- 2Develop, implement, and evaluate program systems for the ACT Team to improve service delivery.
- 3Provide administrative supervision of ACT personnel and coordinate staff meetings, case conferences, and training.
- 4Review referrals, assign caseloads, and redistribute workloads to balance capacity and client needs.
- 5Monitor participant records and progress, ensuring accurate documentation and outcomes reporting.
Requirements8
- 1Master’s degree in psychology counseling social work sociology nursing rehabilitation or related field
- 2At least five (5) years of experience in the Mental Health field and a minimum of 3-4 years supervisory/management experience
- 3Excellent communication skills and highly organized with strong attention to detail
- 4Ability to work independently and as part of a focused team
- 5Valid driver’s license with an acceptable driving record
- 6Availability for local travel and occasional emergency coverage
- 7Must be able to sit for long periods, communicate clearly, and lift or move items as needed
