- 4Maintain caseload providing assessments service planning crisis intervention housing assistance referrals client advocacy
- 5Assist clients with housing applications provide transportation community resource connections
- 6Ensure timely accurate documentation in HMIS WellSky and MyEvolv
- 7Submit authorization requests monitor client eligibility Medicaid billing compliance
- 8Build professional relationships with MCOs
- 9Prepare monthly quarterly reports participate agency community meetings
- 10Hold bilingual Spanish English candidates encouraged apply

Requirements6
- 1Bachelor degree BA BS required
- 2Supervisory and case management experience required
- 3Experience with homelessness populations preferred
