Care Coordinator, Adolescent Mental Health
Overview
As a Care Coordinator at ROWI Teen and Parent Wellness Centers, you own the full coordination of care for adolescent clients, from intake orientation to daily transportation and group facilitation. You work within a supportive, interdisciplinary team of therapists and medical staff, ensuring each client receives consistent, high-quality care. What You'll Do10
- 1Implement client orientation on admission day, including UA screening, PHQ9 assessments, and other directed evaluations.
- 2Supervise clients throughout the day, ensuring attendance at all scheduled groups and activities.
- 3Facilitate assigned groups and activities as delegated by the Facility Manager.
- 4Coordinate the daily transportation schedule, drive clients to appointments as needed, and document all trips in EMR.
- 5Administer and document client urine analysis results in EMR within 24 hours.
- 6Compile shift reports, sharing pertinent client information with the treatment team.
- 7Ensure compliance with Joint Commission standards in all care activities.
- 8Arrange daily client lunches and support facility cleanliness to maintain a safe environment.
- 9Model positive, professional, and caring behavior for staff, clients, and guests, adhering to confidentiality and dress code policies.
- 10Attend staff meetings and complete assigned tasks accurately and on time, including additional duties from management.
Requirements9
- 11+ years working with kids in a therapeutic setting, preferably with junior high or high school students.
- 22+ years driving experience with a valid driver's license and eligibility for agency vehicle insurance.
- 3High school diploma required; Bachelor's degree preferred.
- 4Valid First-Aid & CPR Certification or willingness to obtain.
- 5Strong interpersonal, organizational, and written/verbal communication skills.
- 6Ability to manage time effectively, work with minimal supervision, and maintain clear boundaries.
- 7Proficient in basic computer skills and EMR documentation.
- 8Capable of setting priorities, handling multiple assignments, and working collaboratively in an interdisciplinary team.
- 9Demonstrates tact, initiative, resourcefulness, and a non-judgmental attitude in all interactions.
