Youth Case Manager, Behavioral Health & Family Services
Overview
Join Quality Moments Behavioral Health as a Youth Case Manager and own the delivery of community-based care for youth in Columbus and Pickerington. You will coordinate services, mentor youth, and collaborate with licensed therapists to support emotional and behavioral well-being. This contract role places you directly with families, building trust and resilience in young lives. You will document progress in an EHR system and drive meaningful outcomes from day one.
What You'll Do8
- 1Build trusting relationships with youth and families through consistent, community-based mentoring and support.
- 2Coordinate care with licensed professionals, including therapists and social workers, to align on treatment goals.
- 3Connect families with community resources and services to address social, emotional, and behavioral needs.
- 4Document all client interactions and progress in the EHR system, ensuring accurate and timely records.
- 5Transport youth to appointments, activities, and family visits using reliable transportation and valid insurance.
- 6Model healthy behaviors and positive communication to help youth build resilience and life skills.
- 7Report concerns or changes in client status to the supervising clinician within 24 hours.
- 8Participate in team meetings and case reviews to evaluate progress and adjust care plans.
Requirements7
- 121+ years of age with a high school diploma or GED.
- 20-2 years experience in social work, counseling, or human services (preferred).
- 3Valid driver's license and reliable transportation with current auto insurance meeting state minimums.
- 4Previous use of an EHR system and professional documentation experience.
- 5High commitment to dependability and interpersonal skills.
- 6Passion for improving quality of life for vulnerable populations.
- 7Ability to pass background checks as required by state regulations.
