Overview

You will own care coordination for consumers at Newliving Healthcare, ensuring they receive timely, person-centered treatment. You will oversee Community Support Workers, manage crisis plans, and maintain compliance with DC Mental Health Standards. This role demands a Master's degree in social services and eligibility for a graduate-level license such as LGPC or LPC. You will work onsite in Washington, DC, directly shaping service delivery and staff development.

What You'll Do10

  • 1Assign every new consumer to a Community Support Worker within 48 hours of intake, verifying proper documentation and treatment plan approval by end of business day.
  • 2Conduct interviews, assessments, and evaluations to identify consumer needs for employment, family, and support resources, then connect them to appropriate services.
  • 3Supervise Community Support Workers and administrative staff, reviewing documentation, productivity, reports, and data management to ensure quality and accountability.
  • 4Develop and implement person-centered treatment plans, updating Individual Treatment Plans (ITP) every six months in line with Chapter 34 standards.
  • 5Oversee Crisis Plan, Locus, and Gains assessments prepared by staff, ensuring proper assignment and service delivery for consumers.
  • 6Provide at least 10 billable services weekly, including direct care and crisis intervention during and after hours as assigned.
  • 7Lead onboarding training for new staff, covering compliance with DBH/MHRS regulations and NLHCS policies.
  • 8Coordinate with local community agencies using HIPAA standards to optimize care and maintain confidentiality.
  • 9Conduct follow-up and aftercare planning, and discharge inactive consumers expeditiously to maintain caseload efficiency.
  • 10Assist with Medicaid entitlement applications and recertification tracking for consumers.

Requirements10

  • 1Master's degree in a social services field from an accredited institution.
  • 2Eligibility to sit for a graduate-level license in DC, including LGPC, LPC, or LGMFT.
  • 33+ years of experience in public health or social services, including supportive or personal care duties.
  • 4Knowledge of HIPAA standards for maintaining confidentiality and coordinating care.
  • 5Experience with DC Mental Health Standards, specifically Chapter 34 compliance.
  • 6Proficiency in developing and reviewing treatment plans, including ITPs and ISSPs.
  • 7Supervisory experience overseeing staff and managing documentation and data spreadsheets.
  • 8Ability to provide crisis intervention and after-hours support as needed.
  • 9Experience with Medicaid entitlements and recertification processes.
  • 10Strong communication and collaboration skills for working with community agencies and internal teams.
