What You'll Do8

  • 1Lead client advocacy across treatment, housing, and benefits, ensuring members receive timely services.
  • 2Build and execute individual service plans, updating them as member goals evolve in the first 90 days.
  • 3Conduct on-site and field visits to homes, job sites, and care facilities, providing Medi-Cal enrollment support.
  • 4Coach members in life skills like money management, hygiene, and job readiness, applying age-specific methods.
  • 5Develop financial plans for Representative Payee clients, approving expenditures and tracking budgets.
  • 6Deliver on-the-spot counseling that aligns with program philosophy and member needs.
  • 7Document progress notes and activity logs, maintaining accuracy and compliance with program standards.
  • 8Drive using personal vehicle for outreach, medication pickup, and member transport, ensuring safety in all trips.

Requirements6

  • 1High School diploma or GED plus 4+ years providing direct mental health services, or Bachelors in social work, psychology, or related field with 2+ years relevant experience.
  • 2California driver's license with current insurance and registration, plus reliable vehicle passing safety inspection if over 5 years old.
  • 3Bilingual Spanish skills preferred for client communication.
  • 4Proven ability to coordinate with community resources like housing, AOD, and primary care.
  • 5Strong documentation skills with attention to detail in progress notes and data entry.
  • 6Flexibility for rotational after-hours on-call coverage and weekday schedule (8:30 AM to 5:00 PM).
