Behavioral Health Consultant, Primary Care Behavioral Health (BHC)
Overview
The Behavioral Health Consultant partners with primary care teams to provide brief, evidence-based behavioral health services across the lifespan using the PCBH model. They deliver same-day consultations, brief interventions, crisis assessment, and behavioral health consultations to support whole-person health. Works collaboratively with physicians, nurses, dietitians, and care coordinators to improve access and reduce barriers to care. This role blends clinical care with interdisciplinary teamwork in a community health setting.
What You'll Do10
- 1Build same-day behavioral health consultations within primary care for diverse populations
- 2Design and deliver brief evidence-based interventions (15-20 mins) using CBT MI ACT and related approaches
- 3Assess and address behavioral, emotional, and psychosocial factors impacting health and chronic disease management
- 4Develop collaborative SMART goals and support patient self-management and behavior change
- 5Provide crisis and suicide risk assessment and safety planning when indicated
- 6Coordinate referrals to specialty behavioral health and community resources
- 7Support prevention, early intervention, and chronic disease self-management
- 8Participate in daily huddles, warm handoffs, case consultations and care coordination
- 9Educate medical staff on behavioral health concerns and evidence-based interventions
- 10Maintain documentation for continuity of care and billing requirements
Requirements5
- 1Master's degree in Psychology Social Work or related Behavioral Health field
- 2LPC LCSW or PsyD certification or eligibility
- 3Two years of directly related experience desired
- 4Bilingual English Spanish preferred
- 5Strong knowledge of psychopharmacology and evidence-based behavioral health treatments
