What You'll Do10
- 1Build and provide behavioral health services to children, families, and individuals within the continuum of care
- 2Provide clinical consultation to providers and staff on mental and behavioral health issues
- 3Assess and diagnose using DSM-IV/ICD-9 criteria for patients referred by medical staff
- 4Develop and implement brief, evidence-based, solution-focused interventions and psycho-educational plans
- 5Collaborate with multidisciplinary teams to integrate behavioral health into primary care and ensure timely feedback to referring practitioners
- 6Coordinate crisis triage and on-site intervention as needed
- 7Create and monitor self-management goals for chronic conditions
- 8Educate providers on patient behavioral health issues to improve utilization of interventions
- 9Maintain documentation of behavioral health interventions and track referrals to specialty care
- 10Participate in case staffing and clinical huddles to identify patients who will benefit from BHC services
Requirements5
- 1Master's degree in Psychology or Social Work with two years of related experience
- 2Computer literacy
- 3Bilingual English/Spanish preferred
- 4Certification or eligibility as an LPC, LCSW, or PsyD required
- 5CPR and AED certification per American Heart Association/Red Cross guidelines
