What You'll Do10

  • 1Build and provide behavioral health services to children, families, and individuals within the continuum of care
  • 2Provide clinical consultation to providers and staff on mental and behavioral health issues
  • 3Assess and diagnose using DSM-IV/ICD-9 criteria for patients referred by medical staff
  • 4Develop and implement brief, evidence-based, solution-focused interventions and psycho-educational plans
  • 5Collaborate with multidisciplinary teams to integrate behavioral health into primary care and ensure timely feedback to referring practitioners
  • 6Coordinate crisis triage and on-site intervention as needed
  • 7Create and monitor self-management goals for chronic conditions
  • 8Educate providers on patient behavioral health issues to improve utilization of interventions
  • 9Maintain documentation of behavioral health interventions and track referrals to specialty care
  • 10Participate in case staffing and clinical huddles to identify patients who will benefit from BHC services

Requirements5

  • 1Master's degree in Psychology or Social Work with two years of related experience
  • 2Computer literacy
  • 3Bilingual English/Spanish preferred
  • 4Certification or eligibility as an LPC, LCSW, or PsyD required
  • 5CPR and AED certification per American Heart Association/Red Cross guidelines
