Behavioral Health Care Coordinator, Neurology
Overview
You will own a patient panel of behavioral-health cases in an integrated neurology practice, supporting BHI and CoCM across two locations. Work alongside neurologists, an advanced practice provider, and a consulting psychiatrist, with hands-on training in TMS and biofeedback. You shape a growing neurobehavioral service line and see patients from assessment through intervention. This role stands apart because you help build the program itself.
What You'll Do8
- 1Manage a panel of behavioral-health patients, conducting assessments and follow-ups with PHQ-9, GAD-7, and PTSD screens.
- 2Develop and update care plans with the clinical team, integrating behavioral interventions into neurological care.
- 3Support BHI and CoCM workflows, including psychiatric case reviews and coordination with neurologists and primary care.
- 4Provide brief evidence-based interventions using CBT and motivational interviewing within your scope.
- 5Coordinate TMS patient screening, education, and outcome tracking, plus train in biofeedback protocols.
- 6Build behavioral programs for patients with migraine, chronic pain, sleep disorders, dementia, and Parkinson's disease.
- 7Track clinical outcomes and refine workflows as the service line expands.
- 8Document all care in Epic with timely, accurate notes.
Requirements8
- 1Master's degree in social work, counseling, psychology, or marriage and family therapy.
- 2California registration as ASW, AMFT, or APCC.
- 31+ years in behavioral health or integrated care settings.
- 4Strong patient engagement and communication skills.
- 5Experience with CBT, motivational interviewing, or behavioral activation.
- 6Familiarity with CoCM or BHI models.
- 7Exposure to TMS, biofeedback, chronic pain, or neurology is a plus.
- 8Proficiency in Epic EHR documentation.
