Wound Care Nurse, Wound Care Specialist & Nursing Lead
Overview
The Wound Care Nurse will own and advance wound care services for residents at Rio Vista Post Acute and Rehabilitation, caring for patients at our Phoenix facility. You will lead a wound care team, perform specialized wound assessments, and collaborate with the care team to adjust treatment plans. This role offers direct impact on patient healing, infection prevention, and skin integrity in a skilled nursing setting.
What You'll Do7
- 1Assess and monitor wounds
- 2Provide specialized wound assessments, treatment, and management
- 3Clean and bandage wounds
- 4Educate residents and caretakers on wound care and infection prevention
- 5Round weekly with Wound Practitioner and implement orders
- 6Write orders to expedite wound healing and prevent skin breakdown
- 7Collaborate with resident care team to adjust care plans
Requirements5
- 1Active RN or LPN license in good standing
- 2Current Wound Care Certification required
- 3Experience in skilled nursing, post-acute, or rehab setting preferred
- 4Strong clinical, organizational, and communication skills
- 52 years wound care experience preferred
