VA Benefits Specialist, Home Care Outreach
Overview
You will own veteran outreach for Visiting Angels Plano, driving growth by connecting veterans with VA home care benefits. You will build referral relationships with VA Social Workers, Community Care staff, and Veteran Service Organizations across North Texas. Your work will grow our veteran program and ensure veterans receive the care they earned. This role combines VA system expertise with community engagement, making a direct impact on veterans' lives.
What You'll Do8
- 1Build and maintain relationships with VA Social Workers, Community Care staff, and discharge planners to generate referrals.
- 2Partner with VFW posts, American Legion chapters, and county Veteran Service Officers to expand community reach.
- 3Educate veterans and families on VA programs including Aid & Attendance and Homemaker/Home Health Aide benefits.
- 4Guide veterans through VA referral and authorization processes, ensuring smooth transitions to care.
- 5Deliver presentations at health fairs, community events, and veteran organizations to raise awareness.
- 6Coordinate with internal care teams to enable timely admissions and exceptional service.
- 7Track and document all referral activities, maintaining accurate records in our CRM.
- 8Represent Visiting Angels with professionalism and integrity at all times.
Requirements7
- 12+ years working with the Department of Veterans Affairs, veteran services, healthcare, or home care.
- 2Strong knowledge of VA Community Care, Aid & Attendance, and Homemaker/Home Health Aide programs.
- 3Proven ability to educate veterans and families on VA benefits and eligibility.
- 4Excellent communication and presentation skills for public speaking and relationship building.
- 5Self-motivated with capability to work independently and manage a territory.
- 6Strong organizational and time-management skills to handle multiple priorities.
- 7Valid driver's license and reliable transportation for community visits.
