Overview

You will own the full clinical assessment and treatment planning cycle for clients in our Substance Use Prevention Treatment (SUPT) Program, performing ASAM assessments and managing daily billing in Smart Care EHR. You will report to the Program Manager and collaborate with a team of therapists, licensed vocational nurses, and clinical technicians. Your work directly shapes recovery outcomes for clients across Northern and Southern California. This role stands out for its focus on integrated, evidence-based care that blends Western methods with holistic Eastern traditions.

What You'll Do10

  • 1Conduct ASAM assessments of client needs, strengths, and challenges across mental health, substance use, medical, social, and environmental factors.
  • 2Develop personalized treatment plans using motivational interviewing and supportive counseling, addressing recovery goals and psychosocial rehabilitation.
  • 3Lead psychoeducational groups to teach recovery, relapse prevention, and independent living skills.
  • 4Connect clients to resources such as mental health treatment, medical care, housing assistance, and vocational support.
  • 5Maintain regular client contact, offering support, encouragement, and crisis intervention as needed.
  • 6Collaborate with the treatment team and referral partners to ensure coordinated care and discharge planning.
  • 7Document client progress and services in Smart Care EHR per Drug Medi-Cal guidelines.
  • 8Respond to crises, de-escalating and coordinating with emergency services when necessary.
  • 9Monitor client attendance and treatment responses, adjusting plans as needed.
  • 10Communicate progress in weekly interdisciplinary meetings and evaluations.

Requirements8

  • 1CADC or SUDCC certification or higher (required).
  • 22+ years in substance abuse or behavioral health (required).
  • 3Current CPR and First Aid certification (required).
  • 4Valid California driver's license (required).
  • 5Current health screen and TB clearance (required).
  • 6Strong interpersonal, communication, and listening skills.
  • 7Ability to remain calm in stressful situations.
  • 8Professional conduct and ability to maintain client confidentiality.
