Staff Pharmacist, Home Infusion
Overview
A Doctor of Pharmacy (PharmD) in Gilbert, AZ will own preparation, dispensing, and monitoring of IV and subcutaneous medications for home care patients at a pivotal scale. You coordinate within a multidisciplinary team, ensuring sterile compounding, precise dosing, and patient education. 1+ years of infusion or hospital pharmacy experience is required. What You'll Do7
- 1Prepare specialty pharmaceuticals and IV solutions in sterile environments per state and federal regulations.
- 2Monitor lab values and calculate pharmacokinetic dosing for vancomycin and other drugs.
- 3Oversee TPN adjustments and ensure accurate compounding.
- 4Counsel patients and caregivers on medication use, infusion pumps, sterile technique, and catheter maintenance.
- 5Collaborate with physicians, home health nurses, and internal teams to prevent hospital readmissions.
- 6Ensure compliance with State Board of Pharmacy, HIPAA, FDA, DEA, and Medicare regulations.
- 7Manage pharmacy inventory including refrigerated products and prepare medications for home delivery.
Requirements6
- 1PharmD degree and active Pharmacist license in Arizona.
- 21-3 years experience in infusion or hospital pharmacy with IV room responsibility.
- 3Experience with IVIg preferred.
- 4Knowledge of specialty therapy payer requirements and reimbursement.
- 5Familiarity with CareTend system preferred.
- 6Availability for on-call rotation.
