Speech Language Pathologist, SNF & Per Diem
Overview
You will own speech therapy services for a sub-acute and skilled nursing rehab department in North Reading, MA. You will deliver CCC-level care to residents, working alongside a rehab team that includes occupational and physical therapists. This per-diem role lets you join multiple PRN rosters across local facilities. You will join a therapist-owned company that prioritizes ethical care and offers a supportive education department for ongoing SLP certification.
What You'll Do8
- 1Deliver comprehensive speech therapy assessments and treatments for residents in a SNF setting
- 2Develop and document individualized care plans using Microsoft Outlook and Word
- 3Collaborate with nursing and rehab staff to coordinate patient care and discharge planning
- 4Track patient progress and document outcomes with electronic documentation tools
- 5Adapt therapy schedules to meet facility needs, including flexible weekday and weekend hours
- 6Participate in care conferences and family meetings to update on patient progress
- 7Mentor CFY candidates and support their clinical growth as needed
- 8Maintain compliance with state SLP licensing and CCC standards
Requirements8
- 1Current SLP license or license eligible in Massachusetts
- 2CCC certification from ASHA required
- 31+ years of experience in SNF or sub-acute rehab
- 4Proficiency with Microsoft Outlook, Word, and Excel
- 5Experience with electronic medical records (EMR) for documentation
- 6Strong attention to detail and timely documentation
- 7Ability to work collaboratively in a team environment
- 8Flexibility with work hours, including weekends as needed
