Overview
Service Coordinator at Pathways To Housing DC leads client-centered ACT services for individuals with housing instability and mental health or co-occurring substance use challenges. You will collaborate with a trans-disciplinary team to implement recovery-focused, consumer-driven care. This role operates in diverse environments including streets, shelters, jails, hospitals, and apartments, emphasizing housing stability and community reintegration. The team structure and supported housing model distinguish this role from traditional case management.
What You'll Do9
- 1Build and review an Individual Recovery Plan (IRP) with clients and adjust it regularly
- 2Provide outreach, case management, counseling, advocacy, and services in various environments
- 3Deliver housing, education, training, work opportunities, daily living skills, health and wellness support, relapse prevention, money management, medication support, and social relationships services
- 4Maintain written and computerized records, reports, progress notes, incident records, on-call logs, and psychosocial assessments
- 5Organize and participate in community activities and coordinate referrals to services
- 6Lead groups, workshops, and in-service trainings for clients and colleagues
- 7Meet clients in accordance with MD Medicaid guidelines and collaborate with the team to implement evidence-based practices
- 8Work effectively as part of a team to support client recovery and housing goals
- 9Perform related duties as assigned
Requirements7
- 1Bachelor's degree in social work or related field OR 2 years’ experience in mental health or human services
- 2Patience, creativity, flexibility, compassion, and sensitivity to persons with disabilities and minority populations
- 3Experience in social services, supported housing, or work with persons with disabilities
- 4Excellent communication, writing, and computer skills
- 5Bilingual Spanish preferred
- 6Driver's license and vehicle required
- 7COVID-19 vaccination
